From what has happened, it looks like samtool didn't run when you originally read out the firmware. What I would do is download the ixtreme firmware from xbins and extract it out into a folder. Delete the sam-ofw.bin, then copy your sam-ofw.bin into the 12x folder. Then rename that file from sam-ofw.bin to orig.bin, then run the "Make ixtreme Firmware" file that in that folder. You should get a new .bin file named ix14.bin. Rename that to sam-cfw.bin and put that in the same folder where your sam-ofw.bin came from. Then boot to your usb stick and run the fsam command to flash the drive with the hacked firmware.
